What Is the Hajj Medical Assessment?

The Hajj Medical Assessment is a mandatory health examination required by the Ministry of Hajj and Umrah (MOHU) of the Kingdom of Saudi Arabia for all prospective pilgrims intending to perform Hajj in 2026 (1447H).

This assessment ensures that applicants are medically fit, free from disqualifying chronic medical conditions, and physically capable of performing Hajj rituals safely. A Hajj visa will not be granted unless the applicant has met these medical health requirements.

The core Hajj rituals, performed from 8 to 13 Zulhijjah 1447H (estimated to be from 25 to 30 May 2026), are physically demanding and involve:

  • Long walks ranging from 5km to 20km per day
  • Prolonged standing and movement between designated areas
  • Extreme heat with temperatures between 40 to 45 degrees Celsius
  • Intense crowding

Disqualifying Chronic Medical Conditions

The Ministry of Hajj and Umrah (MOHU) has outlined specific chronic medical conditions that automatically disqualify applicants from performing Hajj due to health and safety considerations. These include:

  • Major organ failure – Renal failure requiring dialysis, cardiac failure with symptoms on minimal exertion, chronic pulmonary disease requiring intermittent or continuous oxygen, advanced liver cirrhosis with hepatic failure
  • Severe neurological or psychiatric disorders – Conditions that impair cognition or are accompanied by significant motor disabilities
  • Advanced age with associated dementia
  • Advanced pregnancy or high-risk pregnancy – Pregnancy during the final trimester, and high-risk pregnancies at any stage
  • Active infectious diseases – Active pulmonary tuberculosis, viral haemorrhagic fevers, and other communicable diseases that pose risks in mass gatherings
  • Cancer under active treatment
  • Mobility issues – Full dependence on wheelchairs, scooters, or constant human assistance

Important note: Common chronic medical conditions such as diabetes mellitus, high blood pressure (hypertension), or high cholesterol (hypercholesterolemia) do not automatically disqualify applicants from performing Hajj, if the condition is well-managed and controlled.

Applicants with such conditions must ensure their health is well-controlled through proper medication, regular monitoring, and follow-up with their doctor leading up to the pilgrimage.

What Does the Assessment Involve?

The Hajj Medical Assessment must be conducted by a registered medical practitioner (doctor) in Singapore. The assessment involves the following steps:

Step 1: In-Person Assessment
Conduct an in-person assessment considering chronic medical conditions and physical capability to perform Hajj rituals safely. No virtual medical assessment is allowed.
Step 2: Online Checklist
Doctor completes the compulsory online Medical Assessment Checklist at go.gov.sg/haj2026mef. Results are submitted directly to MUIS.
Step 3: Medical Health Certificate
Doctor issues a Medical Health Certificate directly to applicants, clearly indicating whether the applicant is medically fit or medically unfit to perform Hajj.

What the doctor will assess:

  • Current health status and existing or past medical conditions
  • Projected health condition over the next six months
  • Physical capability to perform Hajj rituals safely
  • Overall fitness for the physically demanding journey

Generally, no blood tests or laboratory work is required unless deemed necessary by your doctor.

What Should I Bring for the Assessment?

When attending your Hajj Medical Assessment appointment, please bring:

  • Letter of Intent from MUIS and your NRIC for verification
  • Details of existing medical conditions, medications, or mobility issues. Bring recent blood test results if available to show good control of chronic conditions.
  • Your regular doctor's contact information (if applicable)

Understanding Your Medical Health Certificate

After the assessment, your doctor will issue a Medical Health Certificate that clearly indicates one of the following outcomes:

Generally, only one medical assessment is sufficient. You may seek a second medical opinion at your own cost, but the final decision will follow MOHU's medical requirements.

The Medical Health Certificate must include:

  • Doctor's name, MCR number, and signature
  • Clinic name and address (or clinic stamp)
  • Date of assessment

Additional Health Considerations

Mobility and Walking Aids:

Walking sticks may be permitted for light support. However, applicants who are fully dependent on a wheelchair, scooter, or constant human assistance do not meet MOHU's medical health requirements, as Hajj rituals require walking long distances and moving through large crowds.

Vaccination Requirements:

In addition to the medical assessment, applicants must comply with vaccination requirements set by the Saudi Ministry of Health, such as:

  • Meningococcal vaccine
  • Pneumococcal vaccine
  • Seasonal influenza vaccine

MUIS will issue a detailed Health Advisory closer to the departure date with specific vaccination requirements and health recommendations.

If Your Health Changes After Certification:

If your health changes after being certified as medically fit, you must inform MUIS immediately. A reassessment may be required to confirm continued fitness to travel. This measure is to protect your safety and that of fellow pilgrims.
